WHAT:
Donna Walker and Evelyn Santiago’s lives were shattered after Hurricanes Charley and Frances. These two Central Floridians lost their homes and everything in it, but thanks to Florida Hospital Healthcare Systems and Pfizer employees they are getting some much needed help. Walker and Santiago are patients enrolled in Florida Hospital Healthcare Systems’s “Florida: A Healthy State,” a program designed to provide disease management to patients with chronic illnesses. When the nurses with the “Florida: A Healthy State” program found out their patient lost their home they jumped into action. Now the two diabetes patients will receive clothing, Listerine, Benedryl Itch Relief Stick, Bengay Pain Relief Cream, and Neosporin Ointment as a result of Florida Hospital Healthcare System and Pfizer employees’ generosity.
Florida Hospital has a contract with Medicaid to provide education over the phone to patients with chronic illnesses. The program, made possible through a grant from Pfizer, encourages people to become better educated and equipped to manage their health condition more effectively Since the nurses provide care over the telephone, this will be the first time Walker and Santiago meet their caregivers face to face.
WHEN:
Florida Hospital Healthcare System nurses will make the special delivery to Donna Walker at her house on Wednesday, September 15, 2004 at 1:30 p.m. at 1435 King Street, Lot B-23, Cocoa, FL 32922
Evelyn Santiago will receive her much needed supplies on Monday, September 20, 2004 at 1 p.m. at her temporary house on 850 Angela Lane, Kissimmee, FL 34741
